A simple guide to wound care referrals.
Not sure if it's time to involve a wound care specialist? Early referral preserves limbs, prevents hospitalizations, and improves outcomes. Here's a quick reference.
The simple rule of thumb: if a wound hasn't shown clear improvement after about four weeks of appropriate standard care — or isn't on track to heal within roughly six weeks — it's time to refer. When in doubt, refer early. We're glad to co-manage.
When to refer a patient
Consider a referral to Precision Wound Care Specialists when a patient has any of the following:
- A wound not improving after ~4 weeks of standard care
- Any diabetic foot ulcer (refer early)
- Venous, arterial, or mixed lower-extremity ulcers
- Pressure injuries not improving despite offloading
- Signs of infection: increasing pain, odor, or drainage
- Necrotic or non-viable tissue needing debridement
- Exposed bone, tendon, or hardware
- Surgical wound dehiscence
- Recurrent or repeatedly breaking-down wounds
- Comorbidities that impair healing (diabetes, PAD, immunosuppression, prior radiation)
- Any wound that may benefit from advanced therapies
- Limbs at risk — when limb preservation is a concern
How to refer
Helpful to include: patient demographics & insurance, wound location and duration, current treatment, and any relevant history, labs, or imaging. We'll keep you updated on your patient's progress throughout treatment.
Frequently asked questions.
When should I refer a patient to wound care?
A good rule of thumb is the "4-week rule": if a wound hasn't clearly improved after about four weeks of appropriate standard care, it's time to refer. Refer sooner for diabetic foot ulcers, signs of infection, exposed deep structures, significant necrotic tissue, or any wound where a limb may be at risk. When in doubt, refer early — earlier specialized care leads to better outcomes.
What types of wounds do you treat?
We treat chronic and non-healing wounds of all kinds, including diabetic foot ulcers, venous and arterial leg ulcers, pressure injuries, surgical and traumatic wounds, and wounds requiring debridement, skin substitutes, negative pressure therapy, or compression. Limb preservation is a particular area of focus.
Does a patient need a referral to be seen?
We welcome referrals from physicians and other providers, and many insurance plans require one. Patients are also welcome to contact us directly to ask about scheduling. We'll help verify what your specific plan requires.
How quickly will my patient be seen?
New patients are typically scheduled within the same week. For urgent concerns, please call us directly so we can prioritize the visit.
Will I receive updates on my patient's progress?
Yes. We work closely with referring providers and keep you informed throughout your patient's treatment, coordinating care so everyone stays on the same page.
Do you accept insurance, Medicare, and Medicaid?
Yes. We accept all major insurance plans, Medicare, and Medicaid. If you have questions about coverage, give us a call and we'll be happy to help you confirm your benefits.
Do you offer in-home or bedside wound care?
We're actively partnering with several residential communities to bring wound care on-site to their residents. While we don't provide in-home visits as a standard service, we're always glad to talk through each patient's situation individually and work out a plan that fits their needs. Reach out and we'll discuss the options together.
